What to do if you think you’re having an emergency:

Anytime you think you are having a life threatening emergency CALL 911, or your local emergency number. To reach our answering service anytime before or after our business hours call 904.396.0450.

For routine non-emergency appointments:

You may call the appointment desk directly 24 hours a day at 904.517.5702 to leave a message, it will ring several times before going to voice mail and will never be answered by our answering service. During the business hours of 8:30am-11:30 and 12:30pm-5:00pm Monday–Friday you may call our main number at 904.396.0450 x 248.

If you prefer to use our online appointment request:

You must first register through our Secure Patient Portal (CLICK HERE). If you are already registered you may logon to the portal and request an appointment.

What should you bring to your appointment?

Please bring to each appointment a valid Driver's License and/or Picture ID, as well as your current insurance and prescription cards. You should have filled out our new patient paperwork in advance and returned it to our office prior to your first visit with us. (CLICK HERE) to download a copy of our new patient paperwork.

For your safety, it is important that we reconcile your medication list on every visit. Therefore, we ask that you bring with you to each visit every medication and over-the-counter item (including vitamins and herbal supplements) that you take on a regular or occasional basis and whether at your own desire or prescribed by another physician or care provider. We understand that this may be a nuisance, but it's important to understand that many vitamins and herbal supplements interfere with certain medications and may render them ineffective or harmful. If possible, you should have your medical records transferred before your initial visit. Be sure to bring some past records or test results that you have. Some may be copied for our chart and returned to you and other may simply be returned to you.

If you have been in the hospital:

Even though not treated by an IMG physicians during your hospital stay, it is most important for you to schedule an appointment with your primary care physician upon discharge. THis visit will ensure that your medical record is updated and that your medications and any new medical conditions are reviewed and properly monotered.

Or if you have started care with another physician or other healthcare provider since your last appointment with us be sure to let our staff know. Be sure to follow the instructions above regarding bringing in all medications and other drugs.

How long does an appointment take:

Appointment times vary based on individual need. However, generally a first appointment will take approximately one to one and a half hours. As a general rule, subsequent follow up visits are booked in 15, 30, 45 or 60 minute increments based on your doctor’s instruction.

When should I schedule a follow-up appointment?

Your physician will determine how often you need to be seen. Because of the busy nature of our practice, it's best to make your next follow-up appointment at the end of each appointment. Generally, Internal Medicine follow up appointments are booked every 3, 4, 6 or 12 months, based on your personal health condition.
